How to refer your patient 

Due to the complexity of our patient population we ask referring providers to provide as much clinical information as possible prior to scheduling. Helpful information to send includes

  • most recent chart notes pertaining to diagnosis
  • pathology reports
  • imaging discs and reports
  • genetic testing
  • surgical reports
  • color photographs
  • any additional documentation of prior interventions performed
