For your child’s first visit to the Spina Bifida Clinic, you should bring:
- Completed health history and medical information
- Medical records, X-rays and radiologic studies
- Current insurance card and photo identification
- Visit referral, if required by your insurance
- Social Security number of patient and parent
- A list of spina bifida questions you or your child may have
- Any equipment your child uses (e.g., braces, wheelchairs, walkers)
Once your child has met with the clinic’s doctors, therapists and nurse, they will provide you with a treatment plan overview. They also will review the timeline for continuing care of your child.
