If you feel your child would benefit from a visit to the pain management center, please have your primary care doctor call the Children's Direct Physician Access Line, 800.678.HELP (4357), to discuss possible evaluation.
Additionally, patients can be referred from other medical specialists involved in the patient’s care. Medical records from outside facilities, including physician notes, specialty consultations, lab and imaging studies, will need to be faxed to our office prior to scheduling an appointment. Acceptance of patients is at the discretion of the pain clinic team. Patients up to age 18 are accepted.
Your family will then be contacted by our nursing staff in order to set up an appointment. A packet of information and patient questionnaires will be mailed to you. You must have these forms completed at time of your arrival or to avoid delays associated with your clinic visit. Your evaluation time will last between 2-3 hours; please plan accordingly. Please bring shorts and a loose shirt for your child to change into for evaluation. Patients who arrive greater than 30 minutes late will need to reschedule for another day.
Most insurance plans are accepted. You will be asked to provide insurance name, member number and telephone number for pre-authorization/pre-certification when contacted to set up an appointment.
