If you’d like to proceed in applying to be a volunteer at St. Louis Children's Hospital please follow the steps below:
Step One: Complete the online volunteer application.
Step Two: Print and complete required forms. Required forms include:
- Reference Forms (print two to be completed)
- Criminal History Background Check Release Form
- Background Check Authorization Form (complete top half of the Authorization Form only)
If you currently reside in Illinois or have previously resided in Illinois within the past 5 years, please print this additional Illinois Background Check Form.
If you are a volunteer applicant younger than 18 years old, please print this additional Parental Consent Form.
Step Three: Return all of completed required forms at one time to St. Louis Children’s Hospital, Volunteer Services, via mail, fax, or e-mail (as a PDF).
All required forms must be sent by the applicant at one time. We will not accept incomplete forms or forms sent separately. If the required forms are not sent at one time, the individual’s application packet will be considered incomplete and will not proceed through the Volunteer on-boarding process.
*Note: Your volunteer application will not be reviewed until you have submitted your online application, reference forms and background check forms. Please allow 2-4 weeks for this review process.
